Promising Strategies from the California Connected by 25 Initiative

As the California Connected by 25 Initiative (CC25I) comes to an end, the learnings and best practices from this work can both inform and guide how California will implement the recently passed Fostering Connections to Success Act (AB12). CC25I was a six-year $6 million dollar investment to transform county child welfare practices in order to improve outcomes in key areas of the lives of foster youth ages 14 through 24. This report presents the strategies and core focus areas used by the eight participating counties, why these strategies and areas matter and tips for AB12 implementation. Links to technical assistance materials and resources are embedded throughout the report.
